HOSPICE Care

  • Requires Physician order
  • Requires terminal Diagnosis with the life expectancy of 6 months or less
  • Requires 2 Initial Physician Certifications of terminal illness
  • Requires Re-certification by Medical Director q benefit period
  • Requires Informed Consent for Treatment and Medical Records
  • Requires Hospice visits:
    • q 14 days by RN
    • q month by SW
    • q 60 days after Benefit Period 2 by Medical Director/FNP
    • CNA as ordered by IDT
    • Chaplain initial visit then per pt/family
  • Hospice required to provide:
    • Medications r/t terminal illness, anxiety, nausea, pain constipation
    • DME
    • Incontinent supplies
  • Requires Routine, GIP, Respite and Continuous Levels of Care
  • Requires revocation for aggressive treatment related to a terminal condition
  • Goal of care is comfort care through End of Life journey: quality of Life
  • Nurse available 24/7 for emergencies
  • Covered by Medicare Part A

Palliative Care

  • Requires Chronic Ilness(es)
    • Respiratory: COPD, Pulmonary Fibrosis
    • Cardiac Disease: CHF, CAD
    • Diabetes
    • Neurological: MS, Parkinson’s, ALS
    • Cancer: continuing Radiation &/or Chemo
  • Requires Informed Consent for Treatment and Medical Records
  • Requires Initial visit by Medical Director/FNP to discuss Advanced Directives
  • Requires initial visit by SW to discuss Advanced directives
  • Patient may continue any/all aggressive treatment
  • Care provided wherever patient resides
  • Goals of care are Coordination of care between all patient’s physicians and management to prevent re-admission to hospitals, ER’s /Urgent Care
  • Nurse Available 24/7 for emergencies
  • Covered by Medicare Part B
